Pharmacyclics offers a voluntary vision plan provided by Vision Service Plan.
Account through ADP services. This allows you to pay for certain eligible expenses with pre-tax dollars. Deductions for these expenses are subtracted from your gross pay, your taxable income is reduced, fewer taxes are withheld, and your take-home pay may be greater.
The Health Care Spending Account allows you to set aside pre-tax dollars to pay for qualifying out-of-pocket medical, dental, vision and prescription drug expenses, including deductibles, coinsurance payments and co-pays for yourself or your dependents, to a maximum of $4,000 per plan year.
Participants may set aside pre-tax dollars up to $5,000 per plan year ($2,500 if married and filing separately) for dependent care expenses. This includes childcare, elder care, and other eligible dependent care. Elections for these plans are made once a year and, once the election is made, it is irrevocable, unless you have a qualifying "Life Event" - please see next page for examples of these changes.
Employee Stock Purchase Plan
Pharmacyclics offers all employees working 20 hours or more per week, and at least five moths per year the opportunity to purchase PCYC stock at a discount of at least 15% via the Pharmacyclics' Employee Stock Purchase Plan. Employees can contribute up to 20% of their salaries and stock is purchased at the end of each six month period. Open Enrollment periods are April and October.
401(k) Retirement Savings Plan with Company Match
Pharmacyclics offers all employees the opportunity to participate in the Pharmacyclics' 401(k) Plan. The Plan offers investment options through Principal Financial. You will be able to withhold up to 50% of your salary (up to the IRS annual maximum) and the company will match 50% of your contribution up to $1,500 annually.
Basic Life AD&D and Voluntary Life AD&D Insurance
Pharmacyclics provides Basic Life AD&D insurance benefits equal to 2 times your annual salary, rounded to the next higher $1,000 to a maximum of $600,000. This is provided through Lincoln Financial Insurance Group.
Long-term disability insurance
Your income is one of your most valuable assets. Pharmacyclics offers Long Term Disability income protection through Lincoln Financial. If you become injured or ill and are unable to work this program protects you financially.
